If you have an appointment for a physical, then save time at your child's appointment by having the forms ready to go. Download and print the necessary paperwork below, and we'll complete it for you.

Prenatal Consults

Please feel free to call our office if you have additional questions about us. Or, if you prefer, call us today to schedule a prenatal consult to meet the provider and staff.

Upon Arrival

If your baby is born at a hospital, simply tell your obstetrician and the hospital staff that you have chosen West Hills Pediatrics as your pediatrician. Our office will be notified of your baby's birth by the hospital staff. Upon discharge, please contact our office about scheduling your baby's first visit to our office.
